Effect of welding fumes on rhinitis symptoms and nasal patency: Quebec apprentice cohort study

Résumé

Background: Rhinitis symptoms are often reported by workers exposed to occupational sensitizing agents.

Aims and Objectives: We evaluated the relation between exposure to welding fumes and rhinitis symptoms and nasal patency (i.e., nasal airflow).

Methods: Inception cohorts of welding, plumbing and heating apprentices were contacted 7-17years post-apprenticeship. Questionnaires were administered; total serum IgE, spirometry and non-specific bronchial hyper-responsiveness (NSBHR) were measured. Rhinitis symptoms at follow-up were defined as having two or more of the following symptoms: sneezing, runny nose, and nasal congestion. Nasal patency was evaluated with the peak nasal inspiratory flow (PNIF) meter. Evaluation was done in 90 of 330 former apprentices at the Hôpital du Sacré-Cœur de Montréal between 2013 and 2017. Cross-sectional analysis with logistic regression was restricted to male subjects (n=68).

Results: Subjects’ median age was 35 years old. Based on the asthma-specific job exposure matrix, 55.9% of the subjects were currently exposed to welding fumes and gases. The prevalence of low nasal patency (i.e. PNIF < 115 L/min) was 38.2% and rhinitis symptoms was 38.8%. Twelve (17.6%) subjects had both rhinitis symptoms and low nasal patency. The risk of having this joint outcome increased in current smokers, subjects who reported wheezing, as well as subjects with NSBHR, elevated IgE and current exposure (odds ratios 1.85, 1.33, 1.68, 1.39, and 2.79, respectively). None of the association were statistically significant possibly due to small numbers of cases.

Conclusions: Larger study is needed to confirm the effect of welding fumes exposure on rhinitis symptom and nasal patency.
